Summary:
Tower Hamlets Leaving Care Service were interested in starting a website fronted by young people who use the service. It would provide information to young people about the service and what it offers. They required a web site to be designed and incorporate a content management system.
About PLANET KIT KAT:
Leaving Care Service is there to help you plan for your future and to stay with you as you move towards independance. It will help you identify where you want to live and what you plan to do over the next few years. Once you have made those decisions LCS will support you during that time – until you are ready to live independently (generally when you are 21 –24 years old).
As a young person leaving our care all young people will have a:
· Personal Advisor
This person is available to help and support you. They will be allocated as your leaving care worker (but you can have some choice in who it is) .The Personal Adviser will attend your reviews and they will also help you to write your Pathway Plan (explained below). Personal Advisers will help to support you until you are at least 21. They will be responsible for making sure you get what you are entitled to. They should be the person who helps you to get the answers to any questions that you have.
A Pathway Assessment and Plan until you are 21 years old
Together we will complete a Pathway Assessment to help identify your skills now and where you would like to be in the future – where you would like to live, your education, training or employment etc., From this assessment we will help you to complete a Pathway Plan – this is your plan and should as much as possible be completed by you.
This plan will set out a pathway of your choice as you move towards independence. The plan will then be regularly reviewed to take account of any changes that you wish to make along the way.
What else can LCS offer?
There is also a Mentoring Scheme, where you can have somone separate from us - who can spend time with you. They will provide support, give advice and information or just be there to have a good time with. They can help you to do things that you may otherwise not have the confidence to do or they can just be there to talk to. If you are interested in having a mentor then speak to your Personal Adviser or call one of our Mentoring Workers at LCS.
